Laminoplasty Versus Laminectomy and Fusion in the Surgery Approach for Multilevel Cervical Spondylotic Myelopathy: A Current Perspective and Meta-Analysis

چکیده
Background: Cervical spondylotic myelopathy (CSM) is a clinical disease which characterized by degeneration of the intervertebral discs and nearby spinal structures. The goal this study to compare laminoplasty (LP) versus laminectomy fusion (LF) for multilevel cervical in terms safety effectiveness. Methods: This meta-analysis from randomized non-randomized control study. database being used were PubMed, Google Scholar, Cochrane Registry, Embase. authors extensively searched relevant papers comparing efficacy LF LP therapy multilayer CSM. outcome measures recorded visual analog scale (VAS), curvature index (CCI), Japanese Orthopaedic Association (JOA) scores, Nurich grade, reoperation rate, complications, rate nerve palsies. analysis performed using RevMan 5.3. Results: Our includes total 14 papers. pooled mean difference between was insignificant VAS neck pain (p=0.54), post-operative CCI (p=0.24), JOA score (p=0.29), grade (p=0.16) (p=0.21). complication palsies higher group compared (OR 2.60, 95% CI 1.85, 3.64, I2=26%, P < .00001) 3.18, 1.66, 6.11, I2=47%, P=.0005) respectively. Conclusion: demonstrates that majority outcomes surgical treatments CSM utilizing are comparable. However, it discovered superior palsy rate. needs additional validation research more extensive sample-size studies.

Laminoplasty versus laminectomy and fusion for multilevel cervical compressive myelopathy
This is a meta-analysis to compare the results between laminoplasty and laminectomy followed by fusion for the patients with multilevel cervical compressive myelopathy. An extensive search of literature was performed in MEDLINE, Embase, the Cochrane library, CNKI, and WANFANG. The following outcome measures were extracted: the Japanese Orthopaedic Association (JOA) scores, cervical curvature in...

BACKGROUND Cervical laminoplasty (CLP) and posterior cervical laminectomy and fusion (CLF) are well-established surgical procedures used in the treatment of cervical spondylotic myelopathy (CSM). In situations of clinical equipoise, an influential factor in procedural decision making could be the economic effect of the chosen procedure. The object of this study is to compare and analyze the tot...

BACKGROUND There is considerable controversy as to which posterior technique is best for the treatment of multi-level cervical spondylotic myelopathy. The aim of this study was to compare the clinical and radiographic results and complications of laminoplasty (LAMP) and laminectomy (LAMT) in the treatment of multi-level cervical spondylotic myelopathy. METHODS We reviewed and analyzed papers ...

Cervical myelopathy describes a constellation of symptoms and signs arising from compression of the cervical spinal cord (Table 1). Because the presentation of the myelopathic patient can be quite subtle in its early manifestations, the diagnosis may easily be missed or wrongly attributed as a “normal” epiphenomenon of aging. However, because the natural history is typically one of stepwise pro...

Cervical spondylotic myelopathy (CSM) is a common condition. Uninstrumented laminectomy may be complicated by postoperative instability, whereas anterior or posterior decompression with fusion may be associated with stiffness and adjacent segment disease. Cervical laminoplasty, initially oriented towards pediatric patients and ossification of the posterior longitudinal ligament, becomes an inte...
